Angel Air LLC Authorization for Dispersal of Cremated Remains


I/We the undersigned authorize and request that Angel Air LLC take possession of and disperse the cremated remains of ________________________________________________________ (full name of deceased) by scattering them from their aircraft in accordance with all applicable Federal, State and local laws.
I/We understand that once the remains are scattered they are unrecoverable and agree that Angel Air LLC have completed their part of this agreement.
I/We hold harmless and indemnify Angel Air LLC from any and all loss, damage, liability or causes of action, including attorney fees, in connection with the disposition or the identification of the cremated remains of the deceased.
The remains container/s will be disposed of by Angel Air LLC.
The obligation of Angel Air LLC shall be limited to the disposition of the cremated remains as directed herein.
I/We agree that Angel Air LLC are not responsible for any loss or damage occurring during transport by the US Postal Service or other carrier chosen by the undersigned.
I/We certify that I/We have the full legal right to authorize the disposition of the remains of the above-named Deceased.
